Learning, Acting, and Building for Rehabilitation in Health Systems (ReLAB-HS) is a USAID-funded activity that is working to strengthen the integration of rehabilitation and assistive technology into health systems through a scalable approach and is committed to supporting local solutions.
The purpose of this request for quotations (RFQ) is to procure a minimum stock of a variety of quality and simple assistive products to enable the provision of appropriate assistive technology services at the Primary Health Care Level in Swat. These services will be provided by primary health care providers who have received training on assistive products provision and mentorship from rehabilitation providers. ReLAB-HS is opening this RFQ for walking aids and toilet and shower chairs.
RFQ timeline
1.October 14th 2024: RFQ launched
2.October 27th 2024:Questions regarding the technical and administrative requirements should be sent in writing via email to [email protected]. Phone calls will not be accepted.
3.October 30th 2024: Deadline for final bid submissions using the Excel Request for Quotation Form
4.November 15th 2024: Deadline for arrival of sample product in SWAT for physical product in-person evaluation by the procurement team only for vendors that not responded of our previous RFQ in June 2023.
Bids received in response to this RFQ by October,30th 2024 will be first evaluated internally by a ReLAB-HS procurement team comprised of procurement and assistive technology experts to determine compliance with the technical and quality specifications and administrative requirements outlined in the RFQ. If administrative and technical compliance is determined, the bidder will be notified to send the requested sample product (only for vendors that not responded of our previous RFQ in June 2023) to ReLAB-HS at the address of:

The bidder will be required to cover delivery and return shipment fees for the sample product.
Bidder Selection Criteria
Each product will be evaluated against the product specifications noted at the end of this RFQ. Supplier(s) will be selected based on:
•Assistive product specifications (60%)
▪Scoring:
▪Up to 50 points will be given upon sample inspection (see details below on how each sample is inspected)
▪Up to 10 points will be given in line with quality standards reports (e.g., ISO durability or equivalent; see below for each product ISO). Conformance with ISO (or similar) durability and quality test protocols and documentation standards as evidenced by narrative descriptions, photos, and presentation of findings in an organized manner are strongly recommended. The objectivity of testing will be assessed as documented by identification of the laboratory (independent vs. manufacturer managed) that is completing the tests, documenting the results, and signing the report. In addition, the age (date) of test procedures as evidenced by the date of testing recorded in the report will also be taken into consideration. Recent testing results are preferable.
Price and payment terms (30%)

▪Bidders will be evaluated based on the total procurement cost, including quoted delivery fees. The lowest-priced bidder will receive 20 points and the other bidders’ score will be 20 multiplied by the ratio of the lowest bid divided by the quoted price.
Prices should be presented in local currency and USD as the payment will be made in USD, based on the State Bank of Pakistan exchange rate (https://www.sbp.org.pk/dfmd/fem.asp) on the day of submission of the offer.
Offers must remain valid for at least 60 (sixty) calendar days after the deadline of the offer.
▪Payment terms: 10 points for 0% upfront payment, 5 points for 50% upfront payment, and 0 points for 100% upfront payment
•Experience and shortest delivery time (10%)
▪Experience in years: The most experienced bidder will receive 5 points and the other bidders score will be 5 multiplied by the ratio of the most experienced bidders divided by the quoted price. Momentum Wheels for Humanity reserves the right to request references or otherwise verify the stated experience level.
▪Delivery time in days: The shortest delivery time will receive 5 points and the other bidders score will be 5 multiplied by the ratio of the shortest delivery time divided by the quoted price.
It is the supplier's responsibility to ensure that all products and required accessories detailed in the specifications of the bid are delivered by the physical sample review due date and time provided, and that all items are clearly labeled and in working condition.
